Most of us think about October as a time for pumpkin spice everything and new fashion choices. But October is also Breast Cancer Awareness month. Although we’d all like to avoid the topic of cancer, most of us have known someone who has been affected by this terrible disease. So initially, awareness campaigns were centered around educating people about early detection, treatment options, and emotional support for women and the families affected by this disease.

However, many women who are eligible for breast reconstruction are not informed of their options. Breast reconstruction is a procedure (or procedures) that is performed to restore the shape, contour, and symmetry following a lumpectomy, mastectomy or congenital deformities. The types of procedures differ based on the reason for your procedure. Many women find that reconstruction surgery is part of a larger healing process. Restoring to them a body that looks and feels familiar.

Unlike many plastic surgeries, breast reconstruction is not a surgical procedure people seek out simply to improve their aesthetic. No one ever plans to have this surgery. This is a surgery that is part of a larger recovery process that comes after a difficult medical journey. Dr. Larsen and our staff have Atlanta’s top reputation for helping patients through this process. Breast reconstruction often involves multiple procedures performed in stages often beginning at the time of mastectomy, or depending on your treatments they can be delayed until a later date. Generally, breast reconstruction falls into two categories, implant-based reconstruction, or flap reconstruction. In implant-based reconstruction, implants help form the breast shape. In the flap reconstruction surgery, the patient’s own tissue from another part of the body is used to form a new breast.

There are two basic categories for breast reconstruction, but there are many different procedures for this surgery. Both surgeries begin with anesthesia.  First, we will discuss the nipple sparing technique, which is the most common type of breast reconstruction surgery that Dr. Larsen’s performs. 

With breast reconstruction, Dr. Larsen’s goal is always to save the nipple and this technique, using tissue expanders, typically allows for the least amount of scarring and gives comparable results to a cosmetic breast augmentation.

When this technique is chosen, it is usually because after radiation or a mastectomy there is insufficient tissue to support and cover a surgical implant. There are several options for a flap technique that involves using donor muscle and fat from another portion of the patient’s body. Common areas used are the lower abdomen, buttocks or upper thighs. In this procedure, care is taken to maintain blood supply to the relocated tissue. There are many reasons for taking tissue from these different areas of your body. Dr. Larsen will help you determine if this technique is the best for your personal body composition or not. 

For women who do not require radiation, or don’t wish to use a donor site, implant-based reconstruction is a possibility. Although this treatment is an easier recovery than a donor tissue flap technique, it can be a more lengthy process. It involves several office visits over 1-2 months after the placement of the expander. 

In this procedure, a breast implant can be used either in place of donor tissue or in addition to it. Reconstruction with an implant alone usually requires the use of an expander. An expander is a medical device placed within your chest before you receive your implant to help the skin stretch slowly over your newly formed breast. The device is filled with saline and expanded through a port during an office visit. After the expansion is complete, a follow up surgical procedure will be made to exchange the expander for a permanent implant.

Depending on the type of mastectomy you received, you might need nipple reconstruction. This is usually the final portion of reconstruction. There are many techniques used to complete this process. Surgically there are techniques used to fold the skin to create the shape desired followed by tattooing to create the correct pigment on your skin.

Most likely you will wake up from surgery with a cotton padding and breast compression garment. Drain tubes are in place to drain excess fluid away from the incisions. It is common for elastic bandages or compression garments to be put in place to reduce swelling. Most patients spend around 5 days in the hospital following reconstructive surgery. This time allows us to keep you comfortable and monitor all of your medical needs. Following your transition home, we will send you specific care instructions. The healing process will continue for weeks or even months depending on chosen treatments.

Over time skin sensations may return and your scar lines will fade. Most women are extremely pleased with the results of their reconstruction with Dr. Larsen and feel like it is the final chapter in their healing process.

What is nipple reduction surgery?

Breast augmentation surgery is one of the most common plastic surgeries available. But some women are surprised to learn that nipple reduction surgery (or areola reduction) can be performed as an additional procedure or a stand-alone surgery. Nipple reduction surgery involves decreasing the size of protruding or disproportionately large nipples and trimming excess tissue to create an aesthetically balanced breast. 

Many women are uncomfortable with the size and shape of their nipples. Although this is not typically a health concern, it can be distressing for some individuals. A common condition called nipple hypertrophy occurs when the projection or diameter of the nipple is overly tall or wide. Nipple hypertrophy can occur as early as puberty, but can occur during other times of hormonal fluctuation such as pregnancy, during breastfeeding, or after the tissue has experienced trauma. 

We all want to feel comfortable with our bodies, and if you are uncomfortable with the size of your nipples then perhaps surgery is the answer. When performing a nipple reduction, the focus is typically on the height of the nipple, however, the diameter of the nipple can also be corrected during this procedure. 

What are the pros and cons of nipple reduction surgery?

Before any surgery, it’s good to assess the pros and cons. Some considerations for areola reduction surgery are that with surgery you can achieve more aesthetically pleasing nipples, and this can boost a patient’s confidence in the way their body looks and how their clothing fits. 

It should also be noted that after surgery breastfeeding may become difficult. In fact, most surgeons would recommend waiting for this surgery until you are sure you’ve completed breastfeeding. It can also cause possible changes in sensation and results look different for everyone. It is best to talk over any concerns with Dr. Larsen to find out if this procedure is in fact right for you.

Should you pair the surgery with a breast reduction or breast augmentation?

While a nipple reduction surgery can be performed alone it can also be performed in combination with other plastic surgeries. It is most commonly with other breast enhancement surgeries such as reduction or augmentations. Some patients opt for this combination surgery to reduce the amount of time they spend under anesthesia and to cut down on surgical recovery time. 

When combining a breast augmentation with areola reduction procedures, an incision is typically made around the areola. This will be used to alter its size and most surgeons can use this same incision to place implants. This type of incision is called a periareolar incision. 

Without a combined breast augmentation this procedure will remove portions of the areola through an incision along the outer edges, allowing surgeons to reduce the size and prominence.

Before the surgery, preoperative marks will be drawn on the areola based on the patient’s preferences that they discussed with Dr. Larsen prior to surgery. In an areola reduction stand-alone procedure usually only local anesthesia is necessary. The base and trunk of the areola will be treated with lidocaine and incisions will be made. 

The surgeon will carefully remove the extra skin and sutures will be used to attach the nipple to the base reducing the height. If the width of the nipple is also being reduced skin from the circumference of the nipple will be sutured back together as well. 

After sutures are in place bandages will be placed over the treated areas and the patient will begin the recovery process. Most patients are recovered within 2 weeks of the procedure. Although some lifestyle adjustments will need to be made during recovery most patients can expect to be back to their regular schedule within a few weeks.

One in every eight women will be diagnosed with breast cancer during her life. Although many women are affected by this disease each year, few are aware of reconstructive options after having a mastectomy or a lumpectomy. Types of Breast Reconstruction that Dr. Larsen Performs

Dr. Larsen has experience with both implant surgeries and reconstruction surgeries using Flap Techniques. Both surgeries provide natural-looking results and are popular among patients. Implant surgeries are typically performed by first inserting a tissue expander and slowly expanding the space in your chest to make room for a permanent implant.

Breast Reconstruction Revisions

Sometimes after the initial reconstruction surgery, revision surgery is scheduled. This is a common follow up procedure regardless of whether you had an implant surgery or a Flap Technique surgery. This procedure is intended to fine-tune the reconstructed breasts in order to improve cosmetic appearance. Typically this surgery is scheduled for about 3 months after the initial reconstruction and is an outpatient surgery. 

For implant-based reconstructions, this procedure usually involves the exchange of the tissue expander for the permanent implant and some contouring. For reconstruction surgeries that were done with the Flap Technique, the revision surgery usually involves scar revisions, grafting, and contouring of the reconstructed breasts. Extra attention will be paid to ensure symmetry and match the breasts as closely as possible.
